I understand that Heaven on Earth Lifestyle LLPis not legally responsible for any incidents or accidents that may occur during my stay. I release Heaven on Earth Lifestyle LLP, its staff, and affiliates from any liability in connection with my participation in activities or use of facilities at Kevala Foundation. 

 

I acknowledge that my participation in activities at Heaven on Earth Retreat may involve inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all risks associated with my participation.

 

I certify that I have disclosed all pertinent information related to my personal mental and physical health to the best of my knowledge. I certify that I am physically and mentally capable of participating in the activities at Kevala Foundation, and I will notify staff immediately if I become aware of any condition that may affect my ability to participate safely. 
 

I understand that by acknowledging this consent form is a legally binding agreement and that by acknowledging it and signing it, I waive certain legal rights.

I also give consent for Heaven on Earth Lifestyle LLP to use any photographs, videos, or recordings of me taken during my stay for promotional or marketing purposes.
